INSTRUCTIONS: 
Complete the information; indicate the conference schedule for which you are registering and the price based on your current membership affiliation (membership will be verified). You will be asked to create a user name (email address) and password, which will allow you to revisit your registration information at any time prior to the conference. You will be asked to indicate your choices of sessions, keynote and meal events. Please take time to record your choices so that plans may be made for food and handouts; however, sessions will not be ticketed and access will be limited only by room size and local fire ordinances. 

PAYMENT: 
Attendee registration ends at 12:00 midnight CST, on Wednesday, May 13, 2009.

ONLINE REGISTRATION WITH CREDIT CARD: 
The online registration form requires payment with a credit card. You will receive an email receipt shortly after submitting your registration and payment. 

PAYMENT BY CHECK: 
If payment by check is preferred, do NOT use the online registration. Download a pdf of the registration form or use the registration form from the brochure and mail with your check. Confirmation will be sent to you by email when the registration is received and processed. Be sure to double-check the accuracy of your email address on the form. 

CONFIRMATION: 
Registration will be confirmed by email, so verify your email address is entered correctly. Your email address will not be released to a third party without your express permission. 

CANCELLATION AND REFUNDS: 
If you must cancel your reservation, the full fee less $25 for processing will be refunded only if written notice of cancellations is received by April 1, 2009; however, substitutions may be made at any time.
